This is one of the Very High Altitude (VHA Series) kits from Public Missiles. It comes complete with clear stick-on decals, and has a total of six fins (2 large and 4 smaller fins).
I would consider an Aerotech H123-W for first flight, and then either H242-T or H112-J.
